6.7 inj blow out?

I posted this in the 6.7 forum and never got a response so I'll try here where it's a little busier.

Came back to work and there is a 2012 f-450 6.7 on the floor with the front left inj blown out of the hole. I looked it over and called Ford as I do all the QA/warranty etc here. 17K on it, used as a plow/salter truck. So, after arranging everything I said I had never seen or heard of that before and one of the supervisors said "yeah that's the same as the other one"

WHAT?? Other one? "Yeah we had a sister truck do the exact same thing just at the first of Christmas break"

 

We've got 10 of these and two blow an inj? 20%? Has anybody heard of this or is this a problem I've never heard of?

Sorry, been gone a while and too busy even when I was back.

So, took it to the dealership and they changed it no questions asked. I asked if they saw it before and they said a couple times. Asked if there was a TSB or anything on it and he said there was...something but it was more than a couple weeks so I forget the acronym he used. Anyway, yes they have heard of it and no it's not common. Haven't had any issues with them since including the other trucks we have.
